Kim Warren asks (re:  Six Sigma, balanced scorecard, etc.):  What did they
do that we missed?

If anyone is interested in an article I wrote called "Life Cycle 
of a Silver Bullet", which addresses not how ideas get started 
necessarily, but how they progress from a good idea to the popular
